  This form is to enroll yourselves or your children. Return page 3 and 4 of this application.

• CustClassB

  This form is needed only if you are enrolling children under 18 years of age. This form must be notarized.

• DrctDeposit

  This will be needed to set up a direct deposit to your bank account for dividends.

• Will-TKC

  This TKC Will is only for shareholders who own Class A stock. This form does not apply for Class B shareholders.
