All records of TKC shareholders are kept in individual file folders
in the TKC Anchorage office and maintained in a computer database. Maintaining
over 2,700 files is an ongoing job; especially with the enrollment process
still being performed on Class B shareholders. Any changes we make to
the shareholder records must be backed up with paperwork. Once the required
paperwork has been received the change(s) are made in the computer and
the paperwork is put in the shareholders file.
The content of a shareholder's file is confidential and will only be
released to someone other than the shareholder with their permission,
or a court order or applicable state law.
Address Change
Address
change forms are also available at either the Aniak or
Anchorage office. In addition, any of the following are also accepted:
- Calista Corporation change of address form
- US Post Office change of address form or postcard
- Any handwritten signed and dated note, form, or letter from the
shareholder stating the new address.

Verification Form
If the shareholder is also a custodian, please list each of the wards
so that an address change may be made for them also. Each individual
shareholder, over the age of 18, must provide the change of address
form in writing with their written signature consent. For example, a
husband and wife must both sign the change of address form, or must
fill one out separately. Also, the parents cannot sign these documents
for the child after the child has turned 18.
Enrolling Children
At the 1994 Annual Meeting of Kuskokwim Corporation, Shareholders authorized
Kuskokwim to enroll up to 2,000 eligible applicants for Class B
stock. Download the Class
B Stock Instructions and Application (PDF 76K) Also, please download the following additional form that is required if Class B applicant is under 18 years: "Custodian Consent form". Due to recent changes in Alaska laws, the Corporation’s attorney has advised that we have all custodians (1) complete, (2) sign and (3) have notarized the attached custodian consent form. Please fill out a separate form for each minor.
As a custodian, you have special responsibilities and rights for minor shareholders including voting rights and receiving any dividends or distributions on behalf of the minor, so it is important for us to have this on file.
Name Change
If a shareholder has changed their name TKC accepts any of following
documents as proof of the change:
- A copy of a legal document stating the change
- Marriage certificate
- Divorce decree
- Adoption certificate
- Birth certificate
- Drivers license or state id card showing the change
